CHAPTER 4
Guidelines for Preparing and
Submission of the Programme
Accreditation Report
This chapter contains information to assist the Responsibility Centre in the preparation and submission for
Provisional, Full Accreditation, Compliance Evaluation and interim report of a programme for both the conventional
and ODL Programme. The Responsibility Centre is expected to provide appropriate information with evidence
that support and best illustrate their specific case. The information provided by the Responsibility Centre for its
submission should be brief, concise, and succinct.
4.1 DOCUMENTATION FOR PROVISIONAL, FULL
ACCREDITATION AND COMPLIANCE EVALUATION
The Responsibility Centre is required to submit the documentation listed below for consideration for accreditation:
1. Declaration
Verifies that the information and evidence provided are correct and endorsed by its management. (Refer
Appendix 4.1).
2. Executive summary
Summary to give general overview and also to clarify the key points.
3. Part A: General Information on the Faculty/Academy/Institute/Centre
Institutional profile of the HEP Faculty/Academy/Institute/Centre (Refer Appendix 4.2).
4. Part B: Programme Description
This is information of the Programme to be accredited. (Refer Appendix 4.3).
5. Part C: Programme Standards
information on the Programme to be accredited for all the standards in the seven areas of evaluation.
6. Part D: Self-Review Report
Self-review of the programme based on the information in Part C.
4.2 HOW TO WRITE AN EXECUTIVE SUMMARY
The executive summary should be about 1 to 2 pages (maximum of 3 pages) which gives a general overview of the
programme. It can be used to highlight (strength and/or changes) in the programme or to clarify certain practices
that are unique to the programme.
It is recommended that the executive summary to highlight on these three key points:
1. Background
2. Status on addressing accreditation recommendations
3. Changes since last visit
It should be noted that for Provisional Accreditation, only the “Background” is relevant.
