Tables:
1. There are 2 types of tables. Compulsory guided tables and flexible tables.
2. Compulsory guided tables.
a) This table must follow exactly the recommended format.
b) Slight modification can be carried out based on the specific programmes. However, the “modification”
will usually result in a more complicated table that is more informative.
3. Flexible tables.
a) The format of the table is up to the Programme Owner.
b) However, the table should reflect the requirements.
Flowchart:
1. Illustrate the processes involved.
2. Preferably add in the person in charge (or center/unit/department) involved with that process.
Description of the different types of “Evidence/Appendix”
The evidence/appendix serves as supporting documentation to prove that standards have been met. The following
are the types of evidence/appendix and recommended structure/format. Equivalent evidence and appendices may
be accepted. The purpose of evidence/appendix is beyond the structure/format. Instead, it is about proving that the
standards have been met. However, verbal or informal information is generally not acceptable.
Minutes of meeting: (can also give meeting notes if it is not a formal meeting)
1. The front page (showing the title)
2. The attendance (showing all the members, who is present and absent)
3. The snipping of the relevant information as required.
4. Signature (not applicable for meeting notes)
Survey:
1. The empty survey form. (highlight the relevant question)
2. The processed data (data that is compile into graphs or tables.)
3. The analysis of the processed data.
4. Minutes of meeting discussing the actions related to the surveyed results.
Letters:
1. The letter (highlight the relevant required information)
2. If it is “Letter of Appointment”, then it should include the Terms of Reference (TOR) and duration of
appointment.
Processes, procedures and mechanisms, guidelines, policies, book:
1. If it is a single Arahan Kerja (or a single document on the guideline or policy) completely relevant with the
required information, just attach the whole document/policy/guideline.
2. If it is one part of a book, extract
a) The front cover of the book
b) Only the relevant pages that contains the required information
Forms:
1. The filled up forms (usually at least for 3 consecutive years if available)
2. *Empty forms if that form has yet, filled up because it has not yet reach the stage to fill up the form.
(typically for Provisional Accreditation).
Report:
1. Report of a certain study or visit
Pictures/ snapshot of website
1. Paste it into a word document.
